Documents we notarize
- Advance Healthcare Directive (AHCD) — California's combined living will and healthcare agent designation
- Durable Power of Attorney for Healthcare
- POLST (Physician Orders for Life-Sustaining Treatment) — note: POLSTs are signed by a physician and do not require notarization; we can clarify what your specific document needs
- HIPAA Authorization
- Do Not Resuscitate (DNR) forms — document-specific requirements vary
Hospital and care facility visits
We regularly notarize healthcare documents at OC hospitals, hospice facilities, skilled nursing homes, and assisted living communities. These visits require a bit more preparation:
- Confirm with nursing staff that the patient is alert, coherent, and able to communicate willingness to sign
- If capacity is in question, the notary may need to make an independent assessment — we cannot proceed if we cannot verify voluntary intent
- Know the room number or unit before the appointment
- Have the patient's photo ID available (or a passport if they don't have a driver's license)
California witness requirements
California's Advance Healthcare Directive requires either:
- Two witnesses who are not healthcare providers, not heirs, and not the agent named in the document, or
- A notary public (without witnesses)
If you choose notarization (the simpler path), no witnesses are needed. We'll confirm the exact requirement for your document at booking.
